This program assists health care organizations and health/human service professionals (Physicians, Nurses, Social Workers, Psychologists, Chaplains, Administrative Assistants, Administrators, Managers, Therapists) in enhancing their sensitivities and understanding of those coping with advanced illness and chronic disease. It will provide professionals with new interventions, tools and resources to better serve patients and their families. The use of an Advanced Care Model which integrates care across multiple dimensions transforms care by promoting patient dignity and choice. Conversations with patients and families regarding their health care needs are important in order to support them and their goals.
