Health Disparities

We conduct patient-centered outcomes research and implement our findings in community settings, reducing health disparities among high-risk groups.

Working with clinical and community partners, our department translates research into health care delivery transformation. In the process, we improve outcomes for people subject to health disparities — namely, racial and ethnic minorities, rural and poor populations, and individuals with complex, comorbid conditions.

Through our research, we have established Patient Action Teams, peer support groups whose members convey educational messages to their community and their primary care medical homes.

We emphasize shared leadership with our Patient Action Teams. Our common goals are to:

  • develop systems of care
  • train health professionals
  • increase preventive screenings
  • design interventions to improve self-management

A network of patient teams from largely disadvantaged neighborhoods that partner with the Primary Care Research Institute to help shape medical practice design, research and quality-improvement initiatives.
