How to be a Donor
In grateful remembrance of those whose donations of themselves have served to advance teaching and research at the Jacobs School of Medicine and Biomedical Sciences.
If you are interested in becoming a donor to the Anatomical Gift Program, your first step is to determine whether you are eligible and then complete the Anatomical Gift Program application.
After completion, please forward the application to our office at the address listed.
Upon receipt, we will forward you a Donor Wallet Card, which indicates your registration in the program and should be kept with you at all times.
If you prefer, you may call our office and request an application. Any inquiries regarding the program may be directed to our office at the contact information below.
