How to be a Donor
In grateful remembrance of those whose donations of themselves have served to advance teaching and research at the Jacobs School of Medicine and Biomedical Sciences.
If you are interested in becoming a whole body donor to the Anatomical Gift Program, your first step is to complete and submit the application packet.
Request a packet electronically with the link below. If you prefer, you may call our office and request an application be mailed to you. Any inquiries regarding the program may be directed to our office at the contact information below.
Upon processing the application, we will mail you a Donor Wallet Card, which indicates your registration in the program and should be kept with you at all times. Acceptance into the program is determined at the time of death via eligibility screening.
