COVID-19 Symptom Attestation Form

Guests and visitors to the Jacobs School of Medicine and Biomedical Sciences buildings must attest that they do not have COVID-19 symptoms to ensure the safety and public health of the UB campus and community.

New York state travel advisory exemption for essential workers

Vendors and company representatives coming to UB to:

  1. set up equipment or
  2. participate in an educational lab or public event

are not eligible for New York's travel advisory exemption for essential workers.

If you have traveled from a state on the COVID-19 Travel Advisory list, you must quarantine in New York state for 14 days.
