ACGME-Accredited Fellowship Program Request for an Exceptionally Qualified Candidate

Application Consideration

Applications will be considered through April 1st of each year to start the following academic year (i.e., materials due by 4/1/25 for 2025-26 academic year start date).

This form applies to candidates who have not completed all required pre-requisite clinical education in an ACGME-accredited residency program, an AOA-approved residency program, a program with ACGME International (ACGME-I) Advanced Specialty Accreditation, or a Royal College of Physicians and Surgeons of Canada (RCPSC)-accredited or College of Family Physicians of Canada (CFPC)-accredited residency program located in Canada. With prior UB GMEC (Graduate Medical Education Committee) approval, select ACGME-accredited fellowships allow exceptions to eligibility criteria as described in the program-specific ACGME requirements

Please be aware that only about 50% of exceptionally qualified candidates that have been approved in the last several years have ultimately gone on to successfully complete their programs. The GMEC is looking for individuals who clearly demonstrate excellence and leadership in their fields and are likely to succeed as fellows from day one (this includes supervising and teaching residents).

Complete the form below. It will be reviewed by GME leadership prior to being reviewed by the GMEC. Completed applications must be received at least 10 days prior to the next scheduled GMEC meeting, which are held the third Tuesday of each month.
