Public Service Loan Forgiveness (PSLF)

GME Procedure for PSLF Processing:

Number one.

Please send an email to ubgmehr@buffalo.edu with the subject line, "Request completion of Public Service Loan Forgiveness (PSLF) form." Do not attach a document or include any Category 1 or 2 data in your email, such as Social Security Number. UB email is not secure.

Number two.

Your email will be reviewed, and training dates will be verified to complete page 2 of the PSLF form. Your Social Security Number will not be entered into the document. 

Number three.

An email will be sent to you from ubgmehr@buffalo.edu with the completed document attached. When you receive the document, your next step is to submit it to the PSLF agency.

If you are to have a DocuSign sent for electronic signature, please send it to ubgmehr@buffalo.edu.

We cannot fill out your PSLF form until you are employed with us. For those beginning training with us, you will need to wait until you begin before submitting a form.
