Objectives for Concepts in Patient Medication Safety for the Pharmacist:

1. Discuss the frequency of occurrence of medication errors and the estimated costs to the health care system in the United States.
2. Define medication errors, and common types that occur in pharmacies.
3. Review the common causes of medication errors.
4. Review the potential source of errors such as drug names, abbreviations, packaging and communication.
5. Describe intentional blindness and ways to avoid it.
6. List steps by the pharmacist to avoid errors during dispensing.
7. Outline a protocol a pharmacist should follow in mitigating a dispensing error.
8. Discuss computer prescriber order entry and its effect on medication error rates.
9. Recognize common errors in electronic prescriptions and identify the appropriate interventions needed.
10. Identify reference sources available to pharmacists for more information.
