Use this form to register for a CME ID number to use when attending our activities. Choose a number you will remember as you will need it each time you sign into a CME activity sponsored by UB.

Your CME ID can be four or five digits long: Use the last 4 digits of your social security number (SSN) or another 4- or 5-digit number you will remember.

Registering for CME Activities

CME activities are planned and presented by our education providers. To register for an activity, contact the education provider, who will be listed on the event’s information page in our calendar.

Using Your CME ID

Use your CME ID each time you sign into a CME activity sponsored by UB.

You can begin using your ID immediately after submitting this form. (We will contact you if the ID you have selected is already in use.)

Please use your CME ID number consistently so that your CME record will be accurate.
