Technical Standards for Admission, Retention, and Graduation of Medical Students with Disabilities Acknowledgement

When completing the Jacobs School Supplemental Application, each applicant will be asked to agree with the terms of this document.  

Jacobs School Supplemental Application Fee Payment

We require a $100.00 application fee. Personal checks or cash are NOT accepted and will delay the processing of your application.

Payment by credit card is highly recommended: we accept Mastercard, Visa, and Discover. The Jacobs School Secondary Application will ask if the application fee of $100 has been paid.

If you cannot pay by credit card, please mail a money order or certified check, payable to the "University at Buffalo," in the amount of $100.00. Print your name and AMCAS ID Number on it and send it to:

Please check for possible policy ineligibility BEFORE you submit your application fee. There are no refunds for administrative withdrawals.

Fee Assistance WAIVERS will be honored in accordance with AMCAS decisions. If AMCAS has granted you a Fee Assistance Waiver, you AUTOMATICALLY have a waiver of our $100.00 fee. It is NOT necessary to notify us: your application material contains this information.
