Residency Spotlight Nomination Form

Objective: To improve resident well-being by recognizing and rewarding resident colleagues with exceptional achievements while maintaining a work-life balance.

Eligibility: Nominations can be submitted by fellow-residents or any work colleagues who finds the recipient worthy of recognition. Nominations can be made by completing the nomination form below, explaining why the nominee merits recognition. This resident’s commitment to exemplary service is evident in his/her daily patient, peer, and interprofessional interactions. He/she consistently role models the following award criteria:

• Demonstrates advocacy for patient safety and quality
• Exemplifies caring and compassion
• Promotes a strong physician/patient relationship
• Communicates with clarity and respect
• Collaborates with staff in the delivery of patient care

Nomination Guidelines: 

• Anyone can nominate a Resident for the Recognition.
• Self-nominations are not allowed.
• Individuals may be re-nominated during the course of the year: however, previous winners of the Resident Spotlight are not eligible to be recognized twice during the same Academic Year.
• Nominees must be in good standing with their ACGME program.

*Please include any particular event or patient/co-worker encounter that made this person stand out as exemplary.

Deadline:

• Please have nomination submitted by the 15th of the Month prior to Month awarded.(ex. submit no later than Nov 15th for a Dec 1st nomination).
