HEALTHeOUTCOMES Evaluation

This project is a two-phase evaluation that will assess implementation of the HEALTHeOUTCOMES dashboard, a population health management tool.  Assessment includes mixed-methods evaluation of implementation from the dashboard users, as well as assessment of patient-level outcomes using a cost-conscious methods design to inform future improvements to the tool and support its integration into clinical workflows.
