Medical Certification And Immunization/Vaccination Requirements

Forms required for verification of immunization/ compliance; deadline is July 1 of each year for submission:

  1. Annual Immunization Review (AIR) Form: all students accepted to BTE/MT
  2. Health Background Form (HBF): all new students to UB.

The University at Buffalo requires a health background form at the time of first attendance at the University.  The student is responsible for filing with the UB Student Health Services, a medical record of the medical examination and health history as required by the University, on the appropriate University forms.  Students who are in health related majors have additional requirements. 

Health Related Student Additional Vaccinations and Requirements

Students who are in health-related programs are mandated to have:

  • Completed Annual Immunization Review (AIR) Form prior to starting their program
  • A physical examination prior to attending their program (part of HBF).
  • Proof of vaccination against MMR (measles, mumps, rubella), or immunity by titer
  • A PPD is required within 12 months prior to starting the program.
  • A Hepatitis B vaccine series (3 injections over appropriate time span)
  • Tdap within 10 years
  • A history of Varicella Disease (Chickenpox), Vaccine, or Immunity by titer is also required.
  • Self-attestation completion on the Annual Immunization Review Form (AIR)
  • An Annual Immunization Review (AIR) Form must also be completed prior to each year of the program. Students are recommended to have the meningitis and HPV as appropriate; all decisions should be made upon consultation between the student and the student's medical provider.
  • UB-mandated immunizations can, and sometimes MUST be supplemented with seasonal flu shots or other mandatory shots as required by the internship sites.  Students will be given as much advance notice as possible for compliance.

Annual Immunization Review (AIR) Form, including Self-attestation completion

Deadline: July 1 of each year.

For returning students in the Health Related Professional Schools, including Medical Laboratory Science, an Annual Health Update Form is submitted annually, with a submission deadline of July 1 prior to the upcoming fall semester.  The UB Student Health Services Office will review the Annual Health Update Forms for students and identify those students in compliance by the beginning of each Fall semester.

Student Health Insurance

The University at Buffalo requires all full-time and all international exchange students to have medical insurance coverage. A student may already be covered by health insurance plan; students should have the parents check their policy carefully to verify coverage.

Safety

SUNY Affiliated hospitals require students to comply with the OSHA Standard on Exposure to Bloodborne Pathogens. 

Clinical Rotations Sites and Immunization Compliance

Students on rotations may need to verify compliance with immunizations for each of the hospital sites.  Due to HIPAA privacy, only the student can obtain their own medical records to verify compliance. Students should request copies of their medical records well in advance of clinical rotation placement, make, and keep copies, for verification of compliance.
